SELECT FC CODE OF CONDUCT
This form must be completed for each soccer player, or other Club program participant, and, if the player or participant is under 18-years old, must also be signed by the player’s parent or legal guardian. No player, or other Club program participant, will be allowed to participate in Select FC tournaments, practices, or games without this form having been properly executed, and on file.
As a youth sports organization, Select FC's mission is development. This includes teaching and maintaining sportsman to all youths participating in our club including but not limited to all the values that go along with playing the game and competing honestly and fairly.
A primary goal of Select FC is to run a youth soccer club that develops athletes while encouraging physical, emotional, and mental growth while emphasizing good sportsmanship..
Coaches, players, parents/guardians/relatives, team managers, and spectators pledge to support the above mission. Such individuals will encourage good sportsmanship by demonstrating positive support, encouragement and respect for all participants, coaches, officials (including their calls during the games), staff, spectators and opposing teams at all times.
Coaches, players, parents/guardians/relatives, and spectators agree to:
● Display good sportsmanship at all times.
● Create a positive environment in which the kids can develop, play, and have fun.
● Cheer in a positive and supportive manner for all players, including opponents.
● Refrain from negative or harsh criticism of any player or team’s performance.
● Show respect to the game officials before, during and after the match. This includes abstaining from criticizing the game officials or undermining their authority.
● Avoid embarrassing your own player, team and/or other spectators.
● Show respect for the opponents: players, coaches and spectators which includes abstaining from criticizing or speaking negatively about opponents.
● No defacing of public property.
● Refrain from verbal, written, physical or sexual harassment activities that could be considered harassment, hazing, bullying or damaging property.
Coaches, players, parents/guardians/relatives, and spectators will refrain from any negative, hostile, threatening, vulgar, embarrassing or inappropriate language, conduct or behavior while participating in or watching any match, practice, or event. Profanity, negative, hostile, threatening, foul, vulgar, or inappropriate language, conduct or behavior will not be tolerated. Threats of or actual violence toward players, parents/guardians/relatives, spectators, game officials or coaches will not be tolerated and are unacceptable. Use of tobacco, alcohol or illegal substances around training or games is prohibited.
I am aware that if I do anything illegal or against Select FC's Code of Conduct that I may be prevented from attending practices, matches, and possibly asked to leave the team/organization.
Select FC's Code of Conduct Policy was developed in recognition of the Indiana Soccer conduct guidelines, and, by participating in Select FC, a member club, coaches, players, parents/guardians/relatives, team managers, and spectators agree to abide by the spirit and intent of the Indiana Soccer guidelines.

WAIVER OF LIABILITY AND RELEASE FORM
Select FC Waiver of Liability and Release Form
This form must be completed for each soccer player, or other Select FC program participant.
I, the undersigned, in consideration for my voluntary participation in organized soccer, do hereby willfully acknowledge that my signature below attests to my understanding and agreement that:
• My player status will be kept in good standing. I will not compromise myself in such a way as to do harm to Select FC, knowing that players may be dismissed from participation, with possible loss of payment or dues, for violent conduct or unsporting behavior on or off the field of play. I agree to pay for any and all damages to any property or indemnities cause by me willfully, negligently, or otherwise.
• Soccer is a physical, contact, sport that involves the risk of injury. I assume all risks and hazards associated with voluntary participation in the sport. I am in proper physical condition to participate in soccer practices and games and have no illnesses, disease or existing injury or physical defect that would be aggravated by my participation. I will inform my coach if this status changes.
• I further acknowledge that this risk may involve loss or damage to me or my property, including the risk of death, or other unforeseen consequences, including those which may be due to the unavailability of immediate emergency medical care. I have a current medical consent form in force. I shall wear shin guards, properly fitted and appropriate shoes, and other protective equipment as provided by soccer rules, to all events. I shall not wear any items that are prohibited by the rules or Select FC.
• I have a current, active, personal injury insurance policy in force which covers my participation. Under any conditions, I am responsible for any and all medical expenses arising from my participation, both in practices and matches and while traveling to and from these events. I have the right and responsibility to inspect the equipment and facilities prior to events and, if I believe that anything may be unsafe, I will advise the coach or supervisor of the condition and may refuse to participate. Participation assumes consent.
• I agree to hold Select FC harmless from any and all claims, loss or damage to my personal property, liabilities and costs, including attorney’s fees, as a result of my participation in Select FC soccer, including travel to and from the club tournaments, leagues, matches, or otherwise (including automobile and air travel) or any events incidental to soccer. If the club incurs any of these types of expenses, I agree to reimburse Select FC.
• I authorize my photograph, picture or likeness, and voice to appear in any documentary, promotion (including advertising), television, video, or radio coverage of the league or tournament, without compensation.
• I authorize that an unaltered copy of this form may be generated and given to the officers or directors of other leagues or tournaments in order to allow my participation in their soccer programs, if the form is required and I have requested to participate.
I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue, the United States Soccer Federation, the State Association, Select FC, LaVille Youth Sports, the league and tournament, their associated directors, administrators, officers, board of directors, managers, employees, coaches, referees, trainers, volunteers, sponsors and advertisers, and other agents, estates or executors, including family members, from any and all liability incurred in the conduct of, and my participation in, their soccer programs. This includes owners, lessors, and lessees or premises, municipalities, government agencies, successors, heirs, and assigns.
I have completely read this document and fully understand its contents. I acknowledge that I have given up substantial rights by accepting this document and that I do so voluntarily.
My signature attests to this on behalf of myself and my executors, personal representatives, administrators, heirs, next-of-kin, and assigns.
As the parent and natural guardian or legal guardian of the participant, I hereby agree to the foregoing Waiver of Liability and Release for, and on behalf of, the participant (player/minor) named. I hereby bind myself, the minor, and all other assigns to the terms of Waiver of Liability and Release. I represent and certify that I have the legal capacity and the authority to act for, and on behalf of, the minor in the execution of this Waiver of Liability and Release.
